The main menu features the Japanese title "まぢいらいら某" which translates as: "Steady Hand (Madji iraraibo)" and text below the title "STARTを押すとすぐに始まります作りが雑ですいません" which translates from Japanese to English into "game will start immediately after you press START".

The gameplay is to guide a dot through the maze surrounded by thin walls and to reach its end. The player advanced to the next level after reaching and clicking that "G O A L" button. Once before the player reaches the end of Level 2, a black-and-white portrait of a baby wearing a sheep costume doing the [[Harlequin Fetus|harlequin fetus]] pose (taken from [[Wikipedia:Deathrealm|Deathrealm]]'s magazine cover) appears in a solid red background, accompanied by a loud edited roar, which is later used in [[Zoeken]]. A beep sound can also be heard when you press start or finish the levels.

In 2005, there was an unofficial English version of ''Steady Hand'' titled ''Tiny Dot Maze''. This version featured minor changes, such as a different menu screen and the "Game Over" message being replaced with "DAMN!". The game's website claimed to reward anyone who completed the supposed "Level 10" with $100, however, upon reaching the end, the player was instead greeted with the [[Hai2u]] image accompanied by a puking sound. Unlike the original, where the screamer appeared just before the end, this version displays the image after the player dies. Afterwards, players are redirected to a page titled "results.php", which displays a message stating "Whoops looks like our maze is a little buggy!!" along with a hyperlink allowing the viewer to replay the game. This site has similarities with other [[Shock site|shock sites]] such as [[PhoneTrace.org|PhoneTrace]] and [[MedicalTracer.com|MedicalTracer]], which provide the same misleading explanations about their site being "buggy" after its shocking content appears.

Like other maze games, this one requires players to navigate through three mazes without touching the walls. Failure to do so will send them back to the beginning, and they must start over. Although the game appears to have only three levels, there is a secret fourth level that reveals the air pump broke. It can be accessed on mobile by tapping the checkpoint or by right-clicking and selecting the checkpoint, effectively cheating. During gameplay, a vectorized artwork of Pikwik Carson's original character appears on the left, which is a recolored version of Axel from the animated television series ''[[Wikipedia:Pikwik Pack|Pikwik Pack]]''. Players are tricked into believing that completing the game without touching the walls will result in the aforementioned character inflating. The game's color scheme and sound effects are very similar to the infamous [[Steady Hand]] game. After the screamer, the game mocks the player for being scared while the Trololo Song plays in the background, eventually repeating in a continuous loop. If the player completes the secret fourth level to avoid the screamer, a congratulatory text appears on the screen.

The two characters start in a school classroom backdrop taken from Scratch’s backdrop library, standing in front of a chalkboard with red curtains on the sides of it. At first, Paddles complains over Sweetie taking some kind of grain all for herself and not wanting to share with him. He proceeds to quack several times at her until she shares, but she doesn't listen. Paddles says, "You asked for it," and the background fades into an open field with several trees, houses, and hills in the background.

The battle begins from here, with role-playing game mechanics similar to that of ''Pokémon'', and Paddles and Sweetie starting with 150% and 200% health, respectively. Paddles uses a rolling attack, thus taking away 25% of Sweetie’s health. The exact opposite happens when Sweetie uses an "energy ball" attack. Paddles then uses a "creepy stare" attack as he is shown facing towards the screen on a black background, accompanied by a creepy reversed music track. Suddenly, his skin and pupils turn red, just for the screen to cut to black. After another second, Paddles is shown with white and red irises and pupils with black outlines around them and cyan outlines around his head and body, while remaining red skin, accompanied by a loud female scream.

After the screamer, the animation cuts back to the field, except Sweetie has lost all of her health. Three seconds later, she disappears with a popping sound and the animation concludes with Paddles becoming happy and closing his eyes.
