The WAD starts with an image of a Cacodemon along with the same scream from the 1988 film ''[[wikipedia:Slugs_(1988_film)\|Slugs]]''. In the menu, The title card is missing and the only difficulty in "Unforgivable". The player starts at Doom I's first level: "Hangar", with a pistol with only 1 bullet. when the player is about to exit, A Cyberdemon appears and kills the player. After dying, The player is given 6 images of dead dogs and penises with the Doom I OST being played inverted and acute. After that, The player is now in Doom II's second level: "Underhalls", with a shotgun with 40 bullets. When the player approaches the brown building in the courtyard, They are spotted by a hostile marine. If the player kills him, They are given an image of static along with text saying "So you get here?". After that, The player is now in a circular room in the dark with a switch in the middle. If the player presses it, They are given an image of a dead body along with text saying "You are dead". There are also crying and screaming sounds from the 2009 creepypasta ''[[Suicidemouse.avi]]'' and a secret demon message in reverse.
